I was amazed at what I discovered and so I began taking niacin myself, to see if its reported lipid lowering, and heart health benefits, would work for me.
I followed the recommendations carefully, not knowing what to expect from the reported “niacin flush,” a transient red rash and burning, tingling sensation that accompanies concentrated niacin ingestion.
I started with only one 100mg capsule, but even that caused me to flush like mad. Gradually, over a period of weeks, I increased my dosage by 100mg increments, until I could finally tolerate 1000mg, three times a day.
Over time, I lost nearly all sensitivity to the feeling of the burning, tingling flush, but I always turned bright red for 20-30 minutes, after taking it. People around me at those times thought I had been out in the sun too long. Other than that, I had no other side effects.
SEEING IS BELIEVING
After a couple of weeks on the full dose of niacin, I checked my own lab tests and found my own total cholesterol levels had fallen to less than 200mg/dL, from my natural level of about 260. And that was when I was fat, not exercising at all and eating all the pizza I could wrap my teeth around. Some months later, I repeated the tests…and they were even better.
Well, those results did it for me. I was convinced that niacin worked as advertised…And, because of the flush, I could actually feel it working. I would visualize it “burning out” any plaque within my coronary arteries.
Of course, at that time, all that was available was the standard, immediate release, crystalline niacin. So, I needed to take 3 grams a day to get the positive results I had. But I was convinced that it was protecting me from my poor eating habits, nonexistent exercise regimen…and my less than excellent draw in the genetic cholesterol lottery.
Later on, I continued my niacin use with extended release formulas–and I still do, to this day (but now that I use extended release niacin, I only take the maximum recommended dose of 2 grams a day).
So that’s my story…and I’m sticking to it! Here’s the truth:

HOW DOES IT WORK?
Niacin has a number of effects as an essential human B complex vitamin (B-3). It helps to produce energy and participates in cellular metabolism and DNA repair… But it’s the lipid lowering effects that concern us, here.
Let me explain how it actually works to protect you from deadly heart attacks, the NUMBER ONE KILLER ON THE PLANET!
Research has shown that niacin stops the breakdown of fat cells within adipose tissue (body fat stores), preventing the release of free fatty acids. This reduces the amount of free fatty acids in the blood stream. It also lowers triglycerides (animal fats in the blood).
This reduction in free fatty acids reduces the liver’s production of VLDL (Very Low Density Lipoproteins) and cholesterol. VLDL particles are the precursors of “bad” LDL cholesterol, the small particles which cause plaque formation (an accumulation of cholesterol within the arterial walls, which narrows the inner diameter and therefore reduces blood flow) in the arteries, leading to atherosclerosis (hardening of the arteries), coronary artery disease and ultimately may result in deadly heart attacks (and/or strokes). So, by this pathway, it lowers LDL levels.
Because of the reduction in the production of VLDL particles, the “good” HDL cholesterol particles tend to increase in the blood stream, which helps to “sweep out” the “bad” cholesterol and return it to the liver for reprocessing. Higher levels of HDL are associated with lower cardiovascular risk.
Because 75-80% of your serum cholesterol is made by your own liver, NOT from your diet, and because niacin reduces liver production of cholesterol, it lowers total blood cholesterol levels substantially.
In short, niacin lowers total cholesterol, lowers LDL (bad cholesterol) and raises HDL (good cholesterol), independently.
NIACIN DOES EVEN MORE….
There is a sub-type of LDL particle, called Lipoprotein (a), which has been shown to be an independent risk factor for coronary artery disease, in addition to the risk associated with increased LDL levels.
There is also another particle called apolipoprotein B-100, or Apo B, which is also associated with increased cardiac risk.
Both of these additional risk particles are thought to be genetically determined, meaning you can’t really control them with diet and exercise.
But guess what? Niacin lowers both of them, too.
Besides lowering all these bad risk factors, some of which may be genetically determined, niacin also raises positive factors, and actually LOWERS your overall risk of cardiac disease.
“What if I‘m already on statins?”
My answer is: sometimes, it’s necessary to add a natural backup, without the side effects of statins (but be sure to get your doctor’s OK, first). Some might say that more than one treatment is redundant, but this is your heart we’re talking about!
Unlike a lot of “natural health” fanatics, I’m not going to tell you that statin drugs are poison, or that you should never take them. They do have their place. If you really need them, and your doctor prescribes them for you, they can be very beneficial.
But they also have their downside. And, as with many other medications, their downside is actually related to their beneficial properties.
Statins work by blocking a specific enzyme (namely HMG CoA reductase). And they are very effective in reducing the 75-80% of the total cholesterol that’s manufactured in your liver. But this same enzyme also promotes the synthesis of CoEnzyme Q10, or CoQ10, which is used by every cell in your body for producing the energy needed for the cells to function.
When you take a statin drug, you not only reduce your LDL cholesterol level and raise your HDL levels, but you also reduce your body’s production of CoQ10.
In addition, statins may cause muscle aches (myalgias) and soreness, which may be dose related. And in very rare cases, these drugs can even cause a very severe and potentially deadly condition (rhabdomyolysis), where the muscle cells break down, releasing their contents (myoglobin) into the circulation, which gums up the kidneys, so they can’t function.
Nevertheless, with proper precautions and the correct indications, statin drugs can be very useful for the prevention and treatment of clinical heart disease.
But you don’t want to run those risks, unless you are already at a higher clinical risk from known atherosclerosis, or diagnosed heart disease, and under a doctor’s care.
In addition, sometimes, statins don’t lower your cholesterol enough, or they lower your “bad” LDL cholesterol, but cannot raise your “good” HDL cholesterol high enough.
That’s when the doctor will often prescribe an adjunct medication, like niacin, to do what the statins alone cannot.
When added to other cholesterol lowering agents, like Red Yeast Rice (a natural HMG CoA reductase inhibitor), or statin drugs (synthetic HMG CoA reductase inhibitors), niacin further lowers bad cholesterol and significantly raises the good HDL and in some cases, this may allow you to take a smaller dose of the statin drug. (Again, this MUST be decided by your doctor.)
If you are not on statin drugs, then you will want to lower your bad and raise your good cholesterol now, so perhaps you won’t need to take those potentially dangerous medications.

Niacimamide is NOT the answer. Studies have shown that it does NOT share the lipid altering effects of niacin.
And as for inositol hexanicotinate, the so-called “flush-free” niacin, there have been contradictory results in such studies that have been done and it does not appear to have consistent lipid lowering effects.
But extended release, or sustained release niacin minimizes the flush, but not the beneficial lipid lowering properties.
If you are already using standard dose niacin, you must begin with only one, 500mg tablet at bedtime. After 4 weeks, you may add another. Each time you want to increase the dose, you must do it gradually.
Though it is often recommended to take the entire dose of extended release niacin at bedtime, I have used equally divided doses, morning and night, for many years, with very good results.
